First go to      https://www.truecaller.com/unlist    

And here you search your number in the search box. (add +your country code and than your number).

Then if Truecaller asked to give one reason why you remove your information then give one reason.

Then verify captcha.

click unlist.

And after 24 hours your number is removed from Truecaller.
